Keep Active is a volunteer delivered integrated care service, run in partnership with the local NHS rehabilitation teams. Keep Active helps older people back to fitness after a fall or illness. Volunteers are matched with clients and visit them weekly for up to nine weeks providing encouragement and support to continue working on their mobility goals. This could include helping with an indoor exercise programme, accompanying someone outdoors or encouraging with stair practice.
This service bridges that important gap between a period of intense healthcare input and having the confidence to access the community once more and reduce NHS readmissions.
This service is free to people 60+ living in the borough of Hammersmith & Fulham who have been referred to us via local NHS community rehabilitation teams and other health teams.
Volunteers are specially trained by rehabilitation therapists and DBS checked.
